I think Fanatics is closed. Don't know of any place in Milwaukee at the moment. (Someone a bit more into it might have a better answer though...)

Appleton= Game Day, Chadwick's University Pub are current hot spots

How many tables are at Gameday and Chadwick's? What is the cost? What are the conditions?

Locations have been removed from Milwaukee for now

Both places have one table. cost is $1. decent tables. Gameday is hot playing on Thursdays and all weekend. I believe Chadwicks is hot on Friday and Saturday. Some one else hop in and clarify this...?

Here is the new Milwaukee foos bar. (it's actually in Waukesha)
they have 3 brand new tables. (silver sides)
There is no coin-op mechanism. You have to "rent" the balls for the night from the bar. I'm not sure how much $$.

Kochanski's Concertina bar in Milwaukee has a Bonzini, it is out on south 37th street. I'm still on the lookout for tables in the more downtown region.

Foosball is hard to find downtown, east side, or bay view. Very unfortunate, but we are always trying to change that. Our place to play foosball is in Waukesha at the Cue Club off of Hwy T. Got a great group of people and some excellent players. We are always looking for new players, and we often handicap our leagues/tournaments to keep everyone on a level playing field.
